What is a hiatal hernia?
Migration of the upper stomach through the hiatus into the chest. Type I (sliding) is the most common; types II–IV are rarer and may require different treatment.
Symptoms: heartburn, acid regurgitation, swallowing difficulty, night cough.
Diagnosis
Gastroscopy, barium oesophagogram and, when needed, manometry / pH study.
Treatment options
Mild cases respond to lifestyle changes and medication. Surgery is considered when symptoms are uncontrolled, for large or complicated hernias.
The surgical approach is laparoscopic hiatal repair + fundoplication (Nissen / Toupet).
How is the surgery performed?
Laparoscopic (5 small incisions) under general anaesthesia.
The diaphragmatic opening is narrowed; the upper stomach (fundus) is wrapped around the lower oesophagus. Hospital stay is usually 2–3 days.
Recovery
A soft diet is recommended for 2–4 weeks. Return to normal activity in 1–2 weeks, heavy work in 4–6 weeks.
