Statement

I have read, reviewed and understood the KVKK Data Protection Notice provided by Op.Dr.Gökhan ATEŞ ('Data Controller').

Under KVKK art. 6, I give my explicit consent to the processing of my health data (medical history, examination findings, laboratory/imaging results, surgical reports, medication, clinical images) for the purposes of medical diagnosis, treatment and follow-up, management of appointments, regulatory compliance and protection of legal rights in case of dispute.

I give my explicit consent to the sharing of my health data — strictly for the above purposes — with the Ministry of Health, SGK, the e-Nabız system, contracted laboratory and imaging centres, contracted insurance companies and technical service providers.

Use of clinical photographs, videos or before/after images for educational, scientific or institutional communication purposes requires a separate written authorisation; this statement does not include such authorisation.
